After you complete the New Patient Contact Form, the practice will receive a message with your information and needs. You will receive a follow-up email from admin (contactus@solreypsych.com) within 3 business days. Prior to the first appointment, intake documents will be sent to you via the secure online platform, Simple Practice. Forms are completed online and saved to your profile. After your first visit, you will also be able to schedule follow up appointments using the Simple Practice online scheduler.
